DESCRIPTION:
The oral glucose tolerance test (OGTT) or two-point glucose tolerance test is performed in patients with hyperglycemia (elevated fasting blood glucose concentration) but not exceeding 8 mmol/L. If the patient has a blood glucose concentration greater than 8 mmol/L, the OGTT may only be performed under physician supervision. Prior to the test, the patient should eat normally (more than 150 g of carbohydrates per day with normal physical activity). Before testing, the patient must fast for 8-14 hours. On the morning of testing, a urine sample is provided and a blood sample is taken while fasting. Then, within 5 minutes, a glucose solution (75 g glucose/0.3 L water) must be consumed. During the test, the patient sits or lies down and must not eat, drink, or smoke. After 120 minutes from the start of the test (from the moment the patient began drinking the solution), a second blood sample is taken.
DETERMINATION:
Sample: SERUM and URINE. Glucose concentration is tested in the first and second blood samples (enzymatic UV method with hexokinase). In the urine sample, the presence of glucose is tested using a test strip. Glucose can be detected in urine when its concentration in blood exceeds 10 mmol/L (depending on the individual’s renal threshold).
CLINICAL SIGNIFICANCE:
Disorders in glucose metabolism can be detected through biochemical analyses: testing glucose in blood or urine. After establishing hyperglycemia and glucosuria (elevated blood glucose levels and presence of glucose in urine), functional tests are undertaken to examine glucose metabolism, including the OGTT. During the OGTT, healthy individuals will have the same or slightly lower blood glucose concentration after two hours compared to the first sample, all within normal value ranges. Individuals with diabetes will have a higher glucose concentration in the second blood sample than in the first.
