According to its place of action, it is an enzyme of digestive juices. Alpha-amylase catalyzes the hydrolysis of the alpha-1,4-glycosidic bond in the polysaccharide molecule to form maltose and glucose from amylose. Apart from maltose, glucose and dextrin, they are formed from amylopectin and glycogen because the enzyme does not act on 1,6 glycosidic bonds. Large amounts of amylase are synthesized in the acini of the pancreas, and to a lesser extent in the salivary glands.<\/p>\n<p>It is also present in other organs, but to a much lesser extent. Amylase is secreted by pancreatic juice into the duodenum; a part passes into the extracellular fluid and thus reaches the circulation, from where it is excreted by glomerular filtration into the urine. Amylase contains a calcium ion in its molecule, which stabilizes the enzyme itself. Chlorine ions activate amylase. Amylase participates in the hydrolysis of polysaccharides at 1-4 bonds, releasing disaccharides and glucose. There are two isoenzymes in the serum: type P amylase from the pancreas and type S amylase from the salivary glands. Both types of amylase isozymes reach the plasma from the site of synthesis. In the urine, where it reaches by filtration through the glomerular membrane, amylase is found in small amounts and therefore shows low catalytic activity.<\/p>\n<p>The catalytic activity of amylase increases in serum and urine during pancreatic disease, especially in the acute phase of the disease, and decreases in duodenal juice. In the course of parotitis (mumps or mumps), the catalytic activity of amylase isoenzyme type S increases in serum and saliva. Increased catalytic activity in serum and urine, and normal in duodenal juice, occurs in the course of renal insufficiency. The catalytic activity of amylase can also be determined in punctates &#8211; ascites and pleural effusion. Biliary diseases, cerebral trauma, and shock may also be accompanied by increased serum amylase catalytic activity.<\/p>\n<p>Amylase isoenzymes originating from the pancreas (isoenzyme P) or from the salivary glands (isoenzyme S) are separated by electrophoresis methods on various supports (agar gel, cellogel). Methods for determining the catalytic concentration of amylase in serum and urine use natural (starch, amylose) or artificial substrates (p-nitrophenyl and chloro-p-nitrophenyl maltosides). In methods with the natural substrate starch, it is possible to determine the catalytic concentration of the enzyme based on: the difference in the coloration of the starch-iodine complex before and after the action of the enzyme, the reduction of the turbidity of the mixture of starch and the examined biological sample, and the determination of reductive substances created by the action of the enzyme. In contrast to the mentioned methods, the only method used today is one in which a dye is attached to starch, amylose or amylopectin, which is released by the action of amylase. After centrifugation, the released color is measured photometrically. By using artificial substrates after incubation with the enzyme, it is possible to continuously measure the product (p-nitrophenol or chloro-p-nitrophenol) at 405 nm. The reference values \u200b\u200bdepend on the principles of the applied methods.<\/p>\n<p><strong>DETERMINATION:<\/strong><br \/>\nColorimetric method for determination of alpha-amylase according to IFCC. Sample: serum or plasma (heparin), urine. Only heparin may be used as an anticoagulant. The stability of the sample is 5 days at a temperature of 4\u00b0C to 25\u00b0C.<\/p>\n<p><strong>CLINICAL SIGNIFICANCE:<\/strong><br \/>\nDetermination of the catalytic concentration of alpha-amylase is used in the diagnosis and monitoring of success in the therapy of acute pancreatitis and for the detection of inflammatory processes of the salivary glands.<\/p>\n<p><strong>RISK FACTORS:<\/strong><br \/>\nReduced values:<br \/>\nThe presence of citrate, oxalate and EDTA interferes, since they bind to magnesium ions and reduce amylase activity. Pyruvate &gt; 1mmol\/L<br \/>\nIncreased values:<br \/>\n\u2013 Drugs \u2013 Gastrectomy \u2013 Surgical procedures (20%) \u2013 Medicines that cause contraction of the sphincter of Oddi \u2013 Methemalbumin (6%) \u2013 Contamination of the sample with saliva \u2013 Oral contraceptives \u2013 Saliva and sweat contain alpha-amylase, and care must be taken to avoid contamination (pipetting through the mouth, contact of the sample with the skin) \u2013 Body position: lying down \u2013 standing (6%) \u2013 Sialography<\/p>\n<p><strong>RESULT:<\/strong><br \/>\nThe reference interval depends on age, gender and work method.
